Disease Management Programs at National Jewish Health
National Jewish Health disease management programs offer a variety of tools and components for participants. Below is a brief description of some of the primary tools offered:
Educational Materials
- Introductory letter describing the program and resources available
- Comprehensive educational booklets from the Understanding series learning booklets
- MedFact educational information sheets
- Spanish and easy to read versions are available
Telephonic Interventions
The number of education calls is determined during the implementation process. A moderate asthmatic may be offered three to six calls per year or more; a severe asthmatic may be offered twice that amount, depending upon client preference.
Once the number of calls is determined, a series of specific and targeted education calls are developed using valuable information gained from the member during the Entry Questionnaire. Each call is designed to address specific educational needs or concerns.
LUNG LINE
- Customized 800 number and greeting established for members
- Staffed by registered nurses who answer questions related to respiratory, allergic and autoimmune diseases from 8:00 am to 6:00 pm Mountain time
- Utilization reports available
Asthma or COPD Self-Management/Action Plan
A symptom-based action plan that allows participants to proactively manage their symptoms or an episode in a timely and effective manner.
Reactive Care Line
Participants will have access to a Care Manager 24 hours a day, 7 days a week for support in managing respiratory symptoms or an episode
Physician Consult Line
Physicians with enrolled patients in the asthma or COPD program are encouraged to consult with a National Jewish Health specialist at any time
Email Communications
Members are encouraged to interact with Care Manager via email if this is their preferred method of communication
Peak Flow Monitoring
- Participants enrolled in the program will receive a peak flow meter and education on benefit and proper use
- Reinforcement of the use of a peak flow meter occurs with each telephonic encounter with the participant
Medical Director Oversight
Medical Director Oversight and case review for all participants on sub-optimal treatment plans or treatment plans inconsistent with national guidelines
Physician Communication
Summary documentation of all National Jewish Health interactions forwarded to physicians and/or health plan case managers within 72 hours of contact
